In suggesting or recommending private duty home care for your patients upon discharge we can help reduce potential readmissions by offering the following:
� Private duty home care services (for those that tend not to be entitled to home care but sometimes afford and might use aide level care).
� Meet patient at hospital to setup care.
� RN visit with medication reconciliation upon arrival in the home (paid with the patient).
� Transportation towards the patient's Physicians office visits within 10
times of discharge to ensure compliance (paid by the patient).
